- Fix buffers sizes when using `MultiByteToWideChar` and `WideCharToMultiByte`.
- input length is now always `-1`, as this will assume null-termination and also ensure it for the output
- output length is the number of array elements (and not the size in bytes as previously specified in some cases)
- Use `str_format` in `fs_storage_path` instead of platform specific functions for better maintainability.
- Implement `fs_is_dir` smarter by using `GetFileAttributesW` on Windows.
- Remove unused `fs_getmtime` that does not work with unicode on Windows in favor of the existing more generalized `fs_file_time`.

Add `UNICODE` and `_UNICODE` defines on Windows and use either the A or the W variant of the Windows API methods explicitly where it was left unspecified.
I supposed this fixes `shell_execute` for unicode paths (e.g. username with unicode) and fixes debug logger output with unicode.
I renamed `logger_debugger` to `logger_win_debugger` to make it clear that it's only available on windows.
I'm using the A-variant for `WSAStringToAddress` and `FormatMessage` because they don't really need to handle unicode, but there is probably a region specific winsock error message containing unicode.
Does not fix unicode commandline arguments.

This drops sending entityex to old client versions, and for stationary pickups not in switch layer (since they don't benefit from entity ex for prediction).
It also slightly changes how m_StartTick is snapped for CLight and CGun when EntityEx is present (always sets it to 0, which could reduce network traffic slightly). However, this might cause misprediction (or invisible lights/guns) for clients who have already changed their version to 15.6, so can omit this part if wanted.

I don't know what the reason for 2a570196ef was, but this reverts it so files are opened case insensitively on Windows again. I assume it was done for consistency so file operations on all operating systems would be case sensitive, but then again I don't understand why it was only being applied when opening files for reading.
While NTFS is actually case sensitive, the normal Windows file API puts a case insensitive abstraction on top.
Previously, if you had multiple files with the same name but different capitalization it would open the file with the correct capitalization, in the unlikely case that you circumwented the file API to create those files. Now it opens the first one it finds non-deterministically, but will also consider a file with different capitalization.
This reverts to the previous behavior, before all the unicode changes, so I suppose it closes#4343.
